Our Services

Blossom Support Coordination is a case management agency devoted to expertly managing Medicaid waiver services, specifically designed to empower individuals with disabilities to thrive within their communities. We orchestrate support across Louisiana’s Region 2, encompassing the picturesque parishes of Pointe Coupee, West Feliciana, East Feliciana, West Baton Rouge, East Baton Rouge, Iberville, and Ascension.

Our support coordination services focus on the following Louisiana Medicaid Waivers:

Early & Periodic Screening, Diagnostic & Treatment (EPSDT)

Provides comprehensive & preventive health care services for children under age 21 who are enrolled in Medicaid.

Children's Choice Waiver (CC)

Provides supplemental support to children through the age of 20 who currently live at home.

Supports Waiver (SW)

Provides services to people (age 18 and older) who would otherwise require the care from an Intermediate Care Facility for the Developmentally Disabled.

Residential Options Waiver (ROW)

Provides services to people (all ages) who would otherwise require the care from an Intermediate Care Facility & nursing facilities to community-based settings.

New Opportunities Waiver (NOW)

Provides services to people (age 3 and older) who would otherwise require care from an Intermediate Care Facility for the Developmentally Disabled to community-based settings.

Adult Day Health Care Wavier (ADHC)

Provides services to adults, age 65 and older, who would otherwise require nursing facility placement.

Community Choices Waiver (CCW)

Provides services to seniors and persons with adult onset disabilities as an alternative to nursing facility care.

What is Support Coordination?

Support coordination is a service that helps individuals and families understand Medicaid waivers, benefits, services, and provider options. Through supportive conversations, assessments, research, and referrals, our support coordinators help those who participate in Louisiana Medicaid Waivers access essential services they need.

Our case managers specifically work to get the support you need based on the Individual Support Plan (ISP) developed. Using this information, the coordinator meets with your chosen service providers to ensure they deliver a healthy environment. This includes regular checkups of providers, creating emergency plans, and ultimately ensuring you receive the resources required to meet all individualized needs. Blossom understands that each person has different needs and goals; our team is committed to creating a personalized plan to meet those needs. Here is how that might work for you:

Develop

When our support coordinators create an Individual Support Plan (ISP), the Medicaid Participant will be aware of the exact steps needed to bridge the gaps of support. Plans like this also help develop milestones, resulting in vital improvements.
We remain proactive instead of reactive, so that our participants are not lacking or missing out on any potential services available to them.

Coordinate

Our team coordinates with you and your support network so that everyone is on the same page. We will make certain that the agreed upon plan is supported by connecting you with providers and ensuring progress remains on track.
We build lasting relationships with our participants and their families through in person meetings, frequent communications, beneficial training, and trustworthy advice.

Need Support Coordination Services?

Individuals with disabilities in Louisiana can receive case management through one of the Home & Community-Based Services (HCBS) waivers as seen above. Those interested in these Medicaid waivers do not need to apply for Medicaid prior to initiating a request as the application process does not begin until after a waiver slot becomes available. Eligible applicants will be prompted to apply for Louisiana Medicaid Long Term Care (LTC) later on on the process.

In an attempt to simplify the procedures, we have laid out the flow of the application process below:

Those interested should begin by contacting their local Human Services District for initial assessment. Once they have determined your need level – and confirm that you indeed qualify – you can then request to be added to the Request for Services Registry (RFSR). Depending on your need level, you may either be placed on a waitlist or possibly even be provided with an immediate waiver slot offer.

Be aware that the Home & Community-Based Services have specific packages and eligibility requirements and waiver opportunities are ultimately dependent upon funding, timing of request, as well as your need as determined through the RFSR.

After finishing your screening with your local Human Services District – and once they have either added you to the waitlist or extended an immediate waiver offer – keep an eye out for an Individual/Family Decision Form that will be sent to the address you provided.

Upon receiving this form, you’ll have the choice to accept or reject the waiver. If you wish to accept the waiver offer, it’s crucial to send back the filled-out form promptly due to the deadlines associated with the offer.

Once your waiver slot is approved and you are connected with your support coordination agency, your next step involves concurrently confirming your eligibility based on financial criteria and, where relevant, meeting the medical or psychological qualifications for an Intermediate Care Facility for the Developmentally Disabled (ICF/DD)
